Founded in 2003, Family Transitions, Inc. began as a small agency providing only intensive in-home services and has now evolved into a qualified and reputable company. 

Family Transitions, Inc. ‘s extensive experience is applied to the challenges of ensuring high quality and cost-effective services. We strive to provide individuals and their families with the services and resources needed to live healthy, fulfilled lives within their families and communities. Our structured community-based programs enhance the lives of both our clients and the community. 

Family Transitions, Inc. plays an active role by participating in civic, charitable, and social organizations. We value the communities that we are involved with and seek to utilize the local workforce for employment opportunities.

Intensive In-Home Services

A family focused-program with counseling provided in the client’s home environment. This program is for children and adolescents from 4-21 with emotional and/or behavioral difficulties, returning from placement and at risk of out-of-home placement. Using the family’s strengths, the program teaches the client and family coping skills to effectively manage stress and crisis situations.

ID Day Support program offers center-based services to individuals with intellectual disabilities. The program offers training, assistance, support, and supervision in addition to a wide array of activities that include but are not limited to assisting individuals with the ability to communicate, improving their social interpersonal skills and personal care development. Individuals who attend the day support program offered by Family Transitions, Inc. are offered the choice to participate in community integration such as bowling, social outings, special trips, shopping, local volunteerism, and the opportunities to engage in physical exercise at the local YMCA. All services provided by Family Transitions, Inc. Day Support Program is person-centered and based on the specific needs of the individuals.
